Coordinator (RN) to join our skilled nursing community in

As our MDS Coordinator, you'll play a key role in assessing
residents, developing care plans, and ensuring everything stays on
track with Medicare and Medicaid guidelines. If you enjoy working

What You'll Be Doing:
--- Gather and assess resident data to help create personalized
care plans.
--- Work closely with the care team to ensure each resident
receives the support they need.
--- Keep everything running smoothly by completing and submitting
MDS assessments on time.
--- Stay ahead of the game by keeping up with MDS regulations and
best practices.
--- Help train and support staff to ensure everyone understands MDS
processes.
--- Be a key part of our quality assurance efforts, always looking
for ways to improve.
--- Support residents and families by keeping communication clear
and professional.
--- Work with the billing team to make sure assessments align with
reimbursement requirements.
--- Lend a helping hand in mentoring new team members and guiding
them through the process.

Qualifications
--- Current Registered Nurse (RN) license in the state of
Colorado
--- Bachelor's Degree in Nursing (BSN) preferred
--- MDS Certification preferred
--- Minimum 2-3 years of experience as an RN in long-term care or
skilled nursing community.
--- Proven experience with MDS assessments and care planning
--- In-depth knowledge of Medicare and Medicaid regulations
--- Strong clinical assessment skills and attention to detail
--- Proficiency in electronic health record systems and MDS
software
--- Excellent communication and interpersonal skills
--- Demonstrated ability to manage time effectively and prioritize
tasks
--- Thorough understanding of the Resident Assessment Instrument
(RAI) process
--- Ability to work collaboratively in a fast-paced healthcare
environment
--- Commitment to ongoing professional development and staying
current with industry trends
