We services adult and geriatric clients on a per visit basis. One year prior clinical experience as a licensed Physical Therapist is preferred. As a home care Physical Therapist, you will be an integral member of the team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.*Summary Responsibilities** Evaluation; initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate times* Developing and revising plan of care* Coordination of services in consultation with home health staff* Discharge evaluation and planning* Documentation of all visits* Supervision of therapy assistants as appropriate*Flexible scheduling** Allows for a healthy work-life balance and still meet the needs of our clients*Company Summary*We have been able to help many, with our vast array of medical services, since 1990.
