Assistant (PTA) to join our growing team and contribute to our

Do:** *Collaborative Care:* Work closely with Physical Therapists
and other healthcare professionals to develop and implement
individualized treatment plans that improve mobility, strength, and
overall well-being for patients in their homes.* *Hands-On
Treatment:* Provide therapeutic interventions such as exercise
programs, gait training, manual therapy, and other physical
modalities as prescribed, ensuring each patient's recovery is on
track.* *Communication is Key:* Regularly communicate with the
Physical Therapist to update them on patient progress, challenges,
and achievements. You'll also educate patients and caregivers,
ensuring they have the knowledge and confidence to continue their
care independently.* *Be the Bridge:* You'll play a key role in
fostering strong, trusting relationships with patients, families,
and caregivers. Your expertise and kindness will be invaluable in
helping our patients regain independence.* *Continuous
Improvement:* Document patient progress, treatment protocols, and
any changes in condition, following both company guidelines and
industry standards to ensure top-quality care.*What You Bring to
the Table:** *Education & Certification:* You have an Associate's
degree from an accredited PTA program and a current state PTA
license. Bonus points if you have previous experience in home
health, but it's not required!* *Skills That Shine:* Strong
interpersonal communication skills, an eye for detail, and the
ability to work independently with confidence.* *Adaptable &

with patients in their homes and have the empathy to meet them
