Job Description:The Behavioral Health Therapist utilizes
clinical expertise to perform psychosocial assessment and develop/
implement a plan of care/treatment plan that provides extraordinary
care for the patient. This position works collaboratively with
patients and the patients' family, healthcare providers, insurers,
community resources, and other involved parties to identify
patients' needs, provide services, coordinate care and transition
plans, and advocate for patients. The Behavioral Health Therapist
performs these duties under the direct supervision of a licensed
mental health professional (LPC, LMFT, LCSW or LP). The
Community-Based Therapist is an integral part of Mount Saint
Vincent's mental health continuum of care. Mount Saint Vincent, a
care site of Intermountain Health, is the Rocky Mountain region's
premier provider of mental health treatment, foster care services,
preschool, K-8 education, and trauma-informed training - all
focused on children and their families. Essential Functions
- Works under the supervision of the assigned LCSW, actively
participating in scheduled meetings, consultations, and chart
reviews as required.
- Acts as a role-model within trauma-informed care to build
strong patient relationships, interpret behaviors, and respond
effectively to patient needs.
- Conducts psychosocial assessments and provide psychotherapeutic
interventions in alignment with the treatment plan.
- Administers assessment tools to screen for behavioral health
symptoms and diagnoses.
- Identifies patients at risk for delays, extended stays,
readmissions, or complex discharge needs and collaborate with care
providers to address issues.
- Manages the discharge planning process, including screening
patient needs, developing discharge plans, and coordinating with
internal and external services for ongoing care.
- Meets productivity standards and expectations.
- Completes accurate and timely documentation in the medical
record, adhering to departmental standards to ensure effective
communication with the care team.

- DocumentationMinimum Qualifications
- Master of Counseling, Social Work, or related field from an
accredited program (degree will be verified)
- Current State Licensure as a LPCC, MFTC, LSW or equivalent
- Excellent written and verbal communication skills
- Proficiency in computer applications
- Positive driving motor vehicle (DMV) record, proof of current
automobile insurance coverage, and a copy of a valid and current
Colorado driver's license
- Basic Life Support and First Aid within 90 days of
employmentPreferred Qualifications
- Experience working in a behavioral health setting
- Bi-lingual in SpanishPhysical Requirements:
- Ongoing need for employee to see and read information, labels,
assess patient needs, operate monitors, identify equipment and
supplies.
- Frequent interactions with patient care providers, patients,
and visitors that require employee to verbally communicate as well
as hear and understand spoken information, alarms, needs, and
issues quickly and accurately, particularly during emergency
situations.
- Manual dexterity of hands and fingers to manipulate complex and
delicate equipment with precision and accuracy. This includes
frequent computer use and typing for documenting patient care,
accessing needed information, medication preparation, etc.
- Ability to regularly lift and / or move up to 50 pounds.
Employees must be physically able to use crisis prevention
intervention techniques including: physical management of a child,
running to prevent a runaway incident. While performing the duties
of this job, the employee is frequently required to stand, walk,
sit, reach with hands and arms; climb or balance; stoop, kneel,

range for this position is listed below. Actual hourly rate
