We are in search of a dedicated Clinical Manager to supervise
skilled nursing care delivery, manage field staff, and ensure

Clinical Management Duties:
Supervise and lead field staff, such as RNs, LPNs, PTs, OTs, STs,
MSWs, and CNAs, to ensure high-quality care provision.
Review and approve field staff documentation, visit time entries,
and address any discrepancies.
Develop and maintain accurate CNA care plans, delegation forms, and
supervisory visits.
Address client concerns by collaborating with leadership to conduct
thorough investigations and reach effective resolutions.
Plan and conduct monthly CNA staff meetings, training sessions, and
continuous educational opportunities (12 CEUs annually).
Conduct competency assessments and annual performance evaluations
for CNAs.
Ensure compliance with Medicare, state regulations, and
requirements from third-party payers.
Field Nursing Responsibilities:
Perform initial and ongoing assessments, including OASIS, to create
care plans.
Deliver skilled nursing care, preventive services, and education to
clients and their families.
Coordinate care with interdisciplinary teams and make referrals to
additional services when necessary.
Conduct essential home health visits and maintain thorough
documentation in the EMR system.

Qualifications:
Graduate of an accredited school of professional nursing; BSN
preferred.
Active RN license in the state.
Minimum of 2 years of nursing experience, with at least 1 year in a
supervisory role preferred.
Strong knowledge of federal and state home health care
regulations.
Proficiency in electronic nursing documentation systems.
Excellent communication, leadership, and interpersonal skills.
