Compensation:

The pay for this position is $56.14 per hour


Benefits are available to full-time employees after 90 days of employment


A 401(k) with company match is available for full-time employees with 1 year of service on our eligibility dates



Highlights:

This position is a contract assignment with potential to hire permanently based on attendance, performance, and business needs


Shift Requirements: Monday - Friday, 8:00 AM - 5:00 PM MST, with on-call rotation


Home Health experience required





Primary Responsibilities:

Perform comprehensive assessments of high-risk members


Collaborate with primary care providers to implement individualized comprehensive care plans with specific interventions to engage members


Identify and reassess cost-efficient, appropriate levels of care


Establish non-medical support systems such as housing and transportation to ensure compliance with treatment plans


Coordinate care transitions, working with physicians, social workers, discharge planners, and claims professionals to transition patients to the appropriate level of care


Engage specialty resources to achieve optimal patient outcomes


Maintain detailed records of clinical, functional, and fiscal outcomes throughout the management process


Identify opportunities for health promotion and illness prevention


Act quickly to prevent adverse patient outcomes, minimizing poor outcomes when possible


Serve as a patient advocate, ensuring privacy and confidentiality are maintained



Requirements:

Education:

Required: Associate or Bachelor's Degree in Nursing or a related field




Licensure & Certification:

Current unrestricted state Registered Nurse license - Required


Case Management certification - Preferred




Experience & Skills:

Ability to analyze complex medical information and make rational clinical decisions


Home Health experience - Required


1+ years of experience with Homecare Homebase (HCHB) Software - Required


1+ years of experience with OASIS (Outcome and Assessment Information Set) - Required


2+ years of experience as a Registered Nurse in a clinical care setting - Required




Other Requirements:

Current driver's license and reliable transportation with insurance - Required
