Job Description:

A company is looking for a Bilingual Medical Assistant Patient CCM Care Coordinator.

Key Responsibilities

Manage and support patients' healthcare needs via phone conversations
Complete individualized patient care plans and perform care management services
Document all communications with patients and their care teams accurately

Required Qualifications

Certified Medical Assistant, Nationally Certified Pharmacy Technician, or LPN with compact license
Bilingual in English and Spanish
At least 2 years of work experience in a related role
Knowledge of healthcare terms and principles of care management
Experience with Electronic Medical Records (EMR) software
